What is the purpose of the affidavit of service?

The purpose of an affidavit of service is a sworn statement of service to be put in writing. It means you're claiming that the information within the document is true and correct to the best of your knowledge.The following should be included in the affidavit of service:

1. Case Reference of the lawsuit parties' names and case number
   
2. The court where the action is filed
3. The name and description of the person who is served and receives the documents
4. Important document list of what papers were delivered
5. The date and time the documents were delivered
6. The precise location where service occurred
7. The full legal name of the person delivering the documents
8. The evidence that an appropriate party has administered oaths and received confirmation
9. Signatures of the server and notary or other official accepting the affidavit
