A write-up form is a document that records how an employee performed in the workplace, either to highlight their achievements or to take disciplinary action against them. If an employee has exhibited bad behavior, a supervisor or manager should write a form, or a co-worker can do so.

What information should be included in employee write-up forms?

In a write-up of an employee, including the following:

Company name.
A description of the employee who is the subject of the article.
As an example, delay can be described as a behavior.
The conduct of the incident (dates, times, written statements of witnesses) must be documented.
An improvement plan that specifies a deadline.

A write-up is important for what reasons?

Keeping track of employee misconduct and major lapses in work policies and standards is accomplished using employee write-up forms. Forms for employee write-ups are helpful for keeping track of poor behavior and assessing employees' less-than-positive characteristics.
