An employee disciplinary action form is use to inform an employee of their failure to meet employment expectation and states the action that will be taken as a result. It is usually stored in employee file as record.
How do you write a disciplinary action form?
Disciplinary action form are usually given to employees as a final warning, it is used when verbal warning has already been given but employee still continues to act in violation or employee offense becomes more serious. Every disciplinary action form should have this basic section in this general order:
- The employee's full name and the date of the write-up.
- The employee's department and supervisor's name.
- Clearly state the type of violation that occurs.
- Clearly state details about the violation.
- Clearly state the decision that will be taken because of the violation .
- The amount the time employee had previously been warned.
- Signature of the employee and employer/ supervisor.
